Is it possible or is there code that would allow the ability to add a picture to a given date. I.E. a picture of a cake on someone's B-day?

It is definitely possible but I've never seen specific code to accomplish it. There was one previous discussion about adding icons to message titles (see the 'Links" section of the Unofficial CalendarScript - Mods and Plugins site). Maybe it will give you some ideas?

Of course, if you allow HTML to be posted in the discription, you could add the HTML code for an image to display in any event you like.
