What is the last version you can upgrade from, can you just install over any previous version, I have a really old version.  Also do I need to repurchase?

How old???

It is NOT possible to "upgrade" from CalendarScript version 2.xx but events from it can be imported into the current version 3.+ calendars once installed, if needed.

Upgrading from any previous 3 version has to be done incrementally (see the following link).
